Choose the appropriate forms packet (1, 2 OR 3) from the above descriptions.
Complete ALL the forms in the packet - make sure that ALL forms that require signatures (player, parent, physician) are signed where indicated.
Write legibly (In emergencies, we cannot call your parents if we cannot read your handwriting)
NOTE - PACKET 1: the PIAA 1-7 form requires a physician's signature.
YOU WILL NOT BE ABLE TO PARTICIPATE UNLESS THERE IS AN AUTHORIZED MEDICAL EXAMINER'S (AME) SIGNATURE ON PART 7 OF THE PIAA FORM!!

PACKET 1 - NEW PLAYER SPRING 2022
If you DID NOT play Ultimate during FALL 2021 AND have not played another sport
at RHS during the 2021-22 school year.
PACKET 2 - GAP PLAYER SPRING 2022
If you DID NOT play Ultimate during FALL 2021 BUT you did play another sport at
RHS during the 2021-22 school year.
PACKET 3 - RETURNING PLAYER SPRING 2022
If you DID play Ultimate during FALL 2021.
CONCUSSION TESTING (required)
All athletes are required to take a baseline testing for concussions every two years.

Email completed CONCUSSION documents (scan or picture) to:
Mr. Kevin Schneider (RHS Athletic Department) at kevin.schneider@rtsd.org
He will generate the online tests for you to take at home as you email the forms. Please know that whoever emails these forms to Mr. Schneider, will be the one to receive the email link to take the test.
The email to take the test will come from IMPACT. The students will need about 30-40 minutes in a quiet area to take the baseline test. Also, it must be taken on a laptop or desktop computer. The baseline test CAN’T be taken on a tablet or phone.
